Appointments

Your first appointment involves a complete evaluation and examination. To help us, we ask that you bring the following to your first appointment:

  1. Written reports of all MRI, CT and X-Rays you may have done, and the actual films if available
  2. A referral from your Physician
  3. Your insurance card and driver's license
  4. An updated list of your medications and their dosage
  5. A completed intake form. Please note that completing this form could take up to an hour, and we recommend that you complete this form at home prior to your arrival to the clinic. Download our intake form.
  6. Please plan on spending at least 60 minutes with Dr.Ghalalmbor and the staff

For follow up appointments you will be given a short follow up sheet to complete on the site prior to being seen by Dr. Ghalambor. Please notify us if you have had any major change in your pain symptoms, general health, allergies and medications.
